How can I request a copy of my health care records?

Submit a written request by email dasha@talesandtrailstherapy.com or through your Sessions Health portal by putting request in a message. Include your name, phone, date of birth, email and address to which you wish the records to be sent. Specify what type of records you would like to receive.
